The five stages of change management are initiation, planning, execution, monitoring and evaluation, and closure.
Initiation is the first stage, and it's when you identify the need or opportunity for change. Planning is the second stage, and it's when you develop a plan to implement the change. Execution is the third stage, and it's when you carry out the plan. Monitoring and evaluation is the fourth stage, and it's when you monitor the progress of the change and evaluate its effectiveness. Closure is the fifth stage, and it's when you close off the project and review the results.
